This 6 Visit Package includes the following: • 1 physical therapy evaluation and 5 physical therapy sessions • Session duration: approximately 60 minutes per session • Valid for 12 months from the date of purchase Payment Terms • The total cost of the Package is $540, payable in full at the time of purchase. • Payment plans are available through Affirm or Klarna • All payments are non-refundable, except as outlined in the cancellation/refund policy.
